Do I need an order from my physician?

Yes, we are required to have an order from your referring physician. Please bring this with you when you come for your exam.

What other information do I need to bring with me?

You will need to bring your insurance card. If your insurance company requires it, you will be asked for your co-payment at time of service.

Do I need to fill out paperwork when I come in?

Yes, there is patient registration information that is required. If you would like to download our patient registration form to complete prior to your visit, please go to our Patient Information section, patient registration area.

If you will be visiting the Women's Imaging Center, please go to that area and download their specific patient registration form. In addition, for mammography please review the information about bringing films from previous mammography studies that may have been performed at another facility.

What insurances do you accept?

We accept most insurance. However, changes occur on a routine basis. If you have questions about your insurance provider, please contact our Billing Department at 850-878-4102.

How do I schedule my exam?

Call 850-878-4127 and one of our scheduling specialists can assist you in scheduling your:

Fluoroscopy procedure
Male ultrasound procedure
PET/Nuclear Medicine study
Interventional Procedure

Call 850-878-6104 and one of our scheduling specialists can assist you in scheduling your:

Mammogram
Female ultrasound procedure
DEXA

What hours are you open?

Diagnostics 7:30 a.m. - 6:00 p.m.
Fluoroscopy 7:30 a.m. - 2:00 p.m.
Interventional Department 7:30 a.m. - 3:30 p.m.
Nuclear Medicine 7:00 a.m. - 6:00 p.m.
PET 7:00 a.m.- 3:00 p.m.
Body Ultrasound 7:00 a.m. - 5:30 p.m.
Vascular Ultrasound 7:30 a.m. - 4:00 p.m.
OB/GYN Ultrasound 7:00 a.m.- 4:30 p.m.
Mammography 6:00 a.m.- 8:00 p.m.
DEXA 7:00 a.m - 5:00 p.m.

I don't understand what my test is:

Please go to our patient information area and click on RadiologyInfo. This area has been designed by two of the largest radiology associations in the country, the American College of Radiology (ACR) and the Radiological Society of North America (RSNA). This will provide you with understandable information about your test or procedure. Should you have questions that were not answered, please contact us.

Are there any special preparations required for my test?

Depending upon the test ordered, the ordering physician may have given you specific preparation instructions. In addition, we have provided preparation instructions in our patient information section. If you have additional questions, please contact our office.

I have a question about my bill, who do I call?

Please call our Billing Department at 850-878-4102.

I need my x-rays for an appointment with my doctor, what do I do?

If your exam was performed on the first floor, call our file room at 850-942-5068 with the following information:

Your name
Your date of birth
Exam performed and date performed
Where you will be taking your films

If your exam was performed on the second floor (Women's Imaging Center) please call our file room at 850-216-3622 with the same information as noted above.

Please note: You must call at least 24 hours before you need your x-rays. Please understand that we get a large volume of these requests daily and this advance notice is necessary so that we may meet your request.

What if I have additional questions that are not addressed here?
